3. I certify that I DO NOT have a restraining order or protective order issued against me by any Court.

 

4. I certify that I have never been convicted of a Violent Misdemeanor or Felony.

 

5. I certify that I am NOT an unlawful user or distributor of any illegal drugs.

 

6. Military Personnel: I certify that I received an honorable discharge from all the branches of the US Armed Forces I served in.
